Evalion is building the AI-native platform for clinical trials. Every drug that reaches patients passes through a process that's barely changed in fifty years — manual coordination, years of delay, billions in cost, and lives lost along the way. We're replacing that machinery with AI agents that source patients, run pre-screening, and monitor compliance in real time, all inside an audit architecture regulators trust.
Founded by Marcelo (serial entrepreneur — his last company scaled from zero to $1B+ revenue), Miguel (ex-Google engineering leader, pioneer of conversational AI agents in healthcare), and Mo (physician with two decades as a healthcare investor and policy advisor). Product and engineering are based in Barcelona; commercial in Boston/NYC. Backed by Javier Olivan (COO, Meta), Fidji Simo (CEO Applications, OpenAI), Guillermo Rauch (CEO, Vercel), and top institutional AI investors. We're assembling a small, elite team with global ambition.
Joining at day 0 means huge ownership: you'll shape the tech stack, draw the architecture, talk to lighthouse customers, and set the cultural tone. You'll partner directly with Miguel on all key technical decisions.

The role
This is intense work. Disrupting a fifty-year-old industry takes stamina and conviction. The reward is proportional: strong compensation, the career arc of founding a defining platform, and the impact of moving medicine forward for real patients. We're looking for someone who would be doing some version of this regardless.
The role is deeply hands-on and AI-first. 60–70% of your time is in code in the first six months, and AI is your primary coding channel — not a tool you sometimes use. We move at the speed we do because we build with AI, not around it. If you haven't already made that shift, this role isn't for you.
MUST-HAVE
* 8+ years building production software — across any stack
* Strong coding skills, with Python as the primary language today (4+ years of Python is enough)
* Strong in architecture — systems design, data flow, service boundaries, tradeoffs under real-world constraints
* Strong in infrastructure — GCP or AWS at production scale, CI/CD, monitoring, security hardening
* Production operations experience — CI/CD, observability, on-call, resolving live incidents
* AI-native engineer — you can point to something real you've shipped where AI wrote most of the code, and you've run AI-powered systems in production
* Barcelona-based or willing to relocate; we work collocated from our Barcelona offices
SOFT SKILLS
* Pragmatic — knows when to build it right and when to ship it fast
* Strong communicator, including with non-technical stakeholders — enterprise clients, site IT teams, auditors, sponsor QA
* Flexible and curious — the AI development ecosystem changes weekly, and you thrive in that environment rather than resist it
NICE-TO-HAVE
* Shipped regulated SaaS end-to-end — healthtech, fintech, medical device, government, or another compliance-heavy domain
* Track record of shipping complex, multi-system integrations with external platforms
* Compliance experience — SOC 2 Type II, HIPAA, 21 CFR Part 11, GCP
* Experience building AI agents in production
* Veteran of early-stage startups or other build-while-flying environments, with clear ownership of something you shipped or scaled
