Job Title: Founding Engineer

Salary: $170K – $250K + 0.5% – 1.5% equity

Company Description: Healthcare AI startup backed by Scale AI angels and founders from Decagon and Modal, SF

Job Description:

This team is automating how the largest hospital systems in America get paid by insurance, using multimodal AI agents across browser automation, voice AI, and API integrations. They have already signed one of the top 3 largest for-profit hospital systems in the country, the founders were early employees at Anduril and have deep networks inside major health systems, and the angels backing them include names from Scale AI, Decagon, and Modal. This is 3 people moving very fast and needing to move faster. They are not looking for their tenth engineer. They are looking for someone who wants to be the world's expert in healthcare AI agents.

Location: San Francisco preferred, but remote-flexible with significant SF presence required

- Own entire products end-to-end as a founding engineer with 0.5% to 1.5% equity, working across browser automation, voice AI, and agentic systems in a market that genuinely has not been cracked yet

What you will do:

- Design and ship multimodal AI agents across browser use, voice AI, text, and API integrations that outperform human workers at complex healthcare billing and authorization workflows

- Work directly with health system operators to understand their workflows, deploy solutions, and drive real financial impact at some of the largest healthcare organizations in the country

- Own features end-to-end from architecture through production, iterate fast based on what you see in the field, and help define what the engineering team looks like as it grows

The ideal candidate:

- 1 to 8 years of experience as a fullstack or product engineer with at least one recognizable company on your resume, a strong CS degree, and a clear track record of shipping product features end-to-end with real user impact

- Proficient in Python or TypeScript with experience in cloud platforms and familiarity with LLMs, voice AI, or agent systems, plus previous customer-facing experience that shows you can work directly with operators and stakeholders

- Visible career velocity with upward progression across roles, currently active as an IC engineer, and genuinely excited about being one of the first people inside a company that has already landed a customer most startups only dream about
